Report format.

While I hope this particular part of the forum doesn't get much use, here is the format in which I would like all reports to be made.

* Field must be filled out.

Picture or Video evidence:

*Rule that has been broken:

*Date in which infraction took place:

Willing witness names:

While a picture or video is not required, it will greatly help us take faster action on the accused.
